Preparing for Your Appointment
Please bring the following items with you:
- All medications you are taking
- Any x-rays
- Your driver’s license or state identification card
- Insurance cards
- A list of questions or any concerns you would like to discuss with your physician
Please download our new patient form. Save time by filling it out at home and bringing it to your appointment. For your convenience, you can either fill it out online and print it out, or print it and fill it out in ink.
Our offices accept most insurance plans. We are happy to discuss billing and insurance in a pre-appointment phone call.
Please call your pharmacy for prescription refills. If you have any questions about any of the medications we have prescribed, please call (912) 537-1014 to speak with a nurse. Allow 48 hours for refill requests to be processed.
